Step 1 Complete Part I- Applicant Information. All information must be legible.
Step 2 Complete Part II- Academic Information. Be sure that you have the appropriate
"course work verification" sheet (s). Complete this section in pencil.
Step 3 Complete Part V- Special Information
Step 4 Meet with your advisor to have Part III- Professional Requirements and Part IV-
Academic Advisor's Recommendation completed.
Step 5 Submit the fully completed application to the Education Department, Administrative Assistant in person or before the deadline.
